That apperance can be made by additional protective ceramic armor plates  
that insert into pockets in the vest.  Also, many times the elastic and  velcro 
straps make an odd pattern on the outer clothing particularly if the  clothes 
over the vest are too snug or if the person is moving in a way that  tightens 
the garment.
