I think this was explained on one of the websites. During public appearances of
various celebrities, political and otherwise, especially overseas, there is
often a sub-titled text service operating. One of the ways they can keep the
subtitles running along with the speaker is a machine that converts speech to
written text. In order to operate properly the machinery requires that someone
on-scene repeat *very clearly* exactly what the target celebrity is saying.
Thus the duplicate wording sometimes overheard from the media types. This would
only be the case where no advanced text of the speech had been provided. But in
such impromptu cases only machine-generated subtitle text could keep up.
It seems clear after these postings that, whatever this "lump" in the Prez's
clothing is, it's been there for some time. The thing to do now is to look back
through the photo archives and see how often it can be seen.
As for presidents having serious medical contidions that are kept from the
public, there are several well-known instances including both FDR and JFK.
There may be nothing to it, or the "pretzel" story may have been a cover. Who
knows? Was the lump there before the pretzel story?
