[FedCom] Uniden Bearcat BC996T and Federal Monitoring

I recently purchased a Uniden Bearcat BC996T and I have been experimenting 
with it for federal monitoring.  I have been running it under the Butel 
ARC996PRO.  I have configured some sessions where I program frequencies as 
P25 conventional only and then check back on the history log to see what 
frequencies were active.  This has worked pretty well for me.  A couple of 
questions though:

-I understand the NEW federal UHF band plan is 406-420 with repeater inputs 
9 mHz higher than the repeater output.  Is that band completely switched 
over yet or are we in a transition period?  Also, once this happens, does 
this mean that 411-414.9875 mHz are simplex only?

-I have noticed some federal frequencies that are BOTH analog AND P25.  I 
seem to have noticed a significant number of analog PL hits on 67.0 hZ when 
some of those are operating with analog modulation.  Does anybody know if 
there is any signifigance to this?  I rarely would get 67.0 hZ used on 
almost anything I have monitored in the past.

-Is there any sort of band plan for 162-174 or can just about any frequency 
be either an input or output on a repeater?
