not entirely correct - having worked presidential campaigns (including McCain) candidates will receive protection when they become the presumptive nominee or when they have a very good chance of becoming the nominee.
In '00 SS attempted to give McCain a detail after New Hampshire but we refused for a couple weeks because of the logistical nightmare involved. Then, going into SC it started getting very ugly and both he and Cindy received threats. At that point they no longer offered the option to decline a detail.
Candidates may also request a protective detail and the request might or might not be granted. If granted the candidate must pay the costs.
