Early Bell binders and Teletype Corp binders were 4-hole.  It was 
felt that the pages would not rip out as easily.  The 4-hole 
practices can be stacked and put in very large expandable volumes 
similar to the style in auto parts stores.  The final solution was to 
add three holes for a total of seven holes.  That way the BSPs could 
be filed in either a 3-hole, a 4-hole, or like all of mine,  in 7 
hole binders.
