OK.  Last night after I went to bed I recalled where I used to hear the term. 
 It was in conjunction with radio station audio patching.  This morning I 
managed to locate the TT-23/SG manual.  Like the SB-1203/UG it has in each 
circuit or loop two looping jacks, a set jack and a misc. jack (not actually in the 
loop).  And a toggle switch to put the local loop supply in or out of the 
circuit.  So in the sense of your description, there are two top jacks.  Sorta.
