I have to agree with the other posters - expresspcb.  I have used them 
now and then for years.  Both for some personal projects and some 
professional ones.  In fact as soon as I have time to lay them out, I 
have a project waiting in the wings where I will have them make about 40 
boards (give or take a few).  Highly recommended!
