I think what you looking for is the same as "RCA" plugs & jacks. I believe it's pretty common to use "phono" and "RCA" to describe the same connectors. The term 1/4 inch plug or jack tends to conjure up a different type, similar to PL-55 and other 1/4 inch diameter types.

For RCA type phono connections, the most commonly available solution is probably a short "Y" adapter cable and can probably be found at Radio Shack or any audio equipment store. I don't think I've ever seen a 2 female to 1 male RCA adapter in a solid piece.
