All of the abrasives/cleaners/polish (Meguiar's 105, wet/dry paper, bonnets, buffer, etc.) I got at my local Pep Boys.  I used Simple Green to pre-clean the plastics.

The Loctite #3926 I stole from my company not my dentist!  (well, I am a co-founder so its not "really" stealing).  Actually we use a number of Loctite light-cure adhesives in the assembly of our medical products, so I contacted our Loctite rep and asked him for a sample of their best product for bonding dissimilar plastics and he gave me a free tube.
I got the polycarbonate screws and nuts and nylon washers from Amazon.com.

I sure second that! Awesome work and thanks for putting together the
documents.

I will contact offlist unless you want to share here, but I would
appreciate sources for some of the compounds, solvents, etc that you
used.  For example, where does one get Loctite #3926 UV-cure other than
stealing it from your dentist?  :-)
