[ARC5] 4-pin to mini tube adapter update

Good morning all,

A quick update.  I assembled the prototype run without issues, so this 
morning I pulled the trigger on a production run of PC boards.  :)

I should have everything in-house by early 2015 at which time I'll have 
kits for sale.
