You are not going to find 500 pf or .005 mfd capacitors these days. The standard values are now 470 pf and .0047 pf. Unless they are concerned with an actual tuned circuit those values will work fine. If, for some reason, you really need 500 pf then you are going to have to put either a 27 pf or 33 pf in parallel with a 470 pf capacitor.
Mouser, http://www.mouser.com , should have what you need. For a 4000 volt capacitor you will probably have to go to a 6000 volt capacitor. But, the higher voltage will work fine.
